pay range
the base pay range for this position is
$125,000.00-$150,000.00 annual
however, base pay offered may vary depending on job-related knowledge, skills, and experience.

position overview
this position is clinic based and will work 3 days a week in a primary care clinic and 1 day a week in a separate location, the palliative care clinic. Both clinics are in the same building. On primary care days, this position will be an extension of an established primary care physician and will see follow ups, acute visits, chronic condition management, and provide preventative care for the patients attributed to that physician. On the palliative care day, the provider will be independent in the palliative care clinic managing their own panel of patients referred for palliative care services.
essential functions: primary care
* assist with diagnosis, treatment, and management of acute and chronic health conditions.
* perform preventive visits and help with the closure of care gaps while working towards the goals of our aco metrics.
* prescribe medications to the extent delegated and licensed.
* generate referrals.
* participate in provider operations council meetings that happen bi-monthly for 30 min each.
essential functions: palliative care
* provide consultation in palliative care, symptom management and supportive care to meet the needs of the patient as requested by the referring physician.
* conduct goals of care conversations to help the patient determine goals of treatment.
* consult with the referring physician, the acute care team, and other involved providers to contribute to the patient’s plan of care.
* prescribe medications including controlled substances to the extent delegated and licensed.

* order diagnostics/treatments as needed.
* prepare and maintain accurate patient records, charts, and documents to support the medical practice and reimbursement for services provided.
* manage patient portal and respond to messages.

* excellent communication skills with patients, families, co-workers, and community partners.

* ability to manage time and multi-task with hard deadlines for completion of duties.
* computer skills including word, powerpoint, excel, and ability to learn electronic medical record.
required education, licensure, certifications, & experience
* licensed to practice in the state of washington as a clinical nurse practitioner.
* experience working in a primary care or acute care setting.
* knowledge of interdisciplinary team concepts
preferred education, certifications, & experience
* experience with palliative care or hospice care.
* certified palliative care arnp.
physical requirements
* sitting: 25%. Standing/walking: 75%.
* occasional lifting/carrying of supplies and equipment weighing between 20-40 pounds.
* occasional pushing/pulling of supplies and equipment weighing between 20-40 pounds.
* occasional climbing of stairs to reach other levels of the building.
* occasional stooping/kneeling/crouching to reach items on low shelves and to get into low cabinets.
* occasional reaching/handling to work with supplies equipment.
* constant fingering to perform work tasks with computer, typewriter, writing, and telephone.
* constant talking/hearing/seeing for all aspects of patient care and administrative duties.
