Job description
under the direction of the youth & family service manager, this position works collaboratively with youth and their families to deliver intensive case management, education related to mental illness and emotional disturbance, and to develop coping skills primarily in community-based settings, such as homes and schools. This position conducts routine needs assessments, completes recovery plans, assists youth and their families with accessing community resources, and initiates crisis supports when needed. Services are provided through an electronic health record and in collaboration with a multidisciplinary recovery team, including psychiatrists, nurses, counselors, and family partners with lived experience.
services should promote recovery and wellness, community integration, stable living, and an improved quality of life. Success in this role involves a passion for working with youth with severe emotional needs, knowledge of psychotherapeutic medications, case management, substance use treatment, crisis services, community resources, and evidence-based practices in behavioral health. The ideal candidate demonstrates initiative, innovative thinking, patience, resiliency, advocacy skills, service coordination, mental health assessment, and collaborative recovery planning, including serving youth with substance use needs.
the person in this position should work effectively both independently and as part of a team, prioritize needs during crises, participate in meetings and training, and perform other duties with a positive attitude.
this is a relief pool position with flexible hours and schedule.
salary: $28.55 – $30.30/hr
applicable differentials:
* $0.75/hr bilingual
* $1.00/hr master’s degree
locations: caldwell, gonzales, guadalupe
minimum qualifications: bachelor’s degree in a human services field; comprehensive knowledge of mental health diagnoses; ability to work as part of a team in a fast-paced treatment setting; valid texas driver’s license with maintained approved driver status.
preferred qualifications: master’s degree and/or clinical license in psychology, education, social work, or related field; one year of work or practicum/clinical experience in intensive case management or psychosocial rehabilitative services; bilingual/spanish-speaking; experience with electronic documentation.
