under the direction of the youth & family service manager, this position works collaboratively with youth and their families to deliver intensive case management, education related to mental illness and emotional disturbance, and to develop coping skills primarily in community-based settings such as homes and schools. Responsibilities include conducting routine needs assessments, completing recovery plans, assisting youth and families with accessing community resources, and initiating crisis supports when needed. The role is supported by an electronic health record and involves collaboration with a multidisciplinary recovery team including psychiatrists, nurses, counselors, and family partners with lived experience.
services provided should promote recovery and wellness, community integration, stable living, and improved quality of life. Success in this position involves a passion for working with youth with severe emotional needs, knowledge of psychotherapeutic medications, case management, substance use treatment, crisis services, community resources, and evidence-based practices. The person should be proactive, innovative, patient, resilient, skilled in advocacy, service coordination, mental health assessment, and collaborative recovery planning. Ability to work independently and as part of a team, prioritize during crises, participate in meetings and training, and perform other duties with a positive attitude are essential.
this is a relief pool position with flexible hours and schedule.
salary: $28.55 – $30.30/hr
applicable differentials: $0.75/hr bilingual, $1.00/hr master’s degree
locations: caldwell, gonzales, guadalupe
minimum qualifications: bachelor’s degree in human services, knowledge of mental health diagnoses, valid texas driver’s license.
preferred qualifications: master’s degree or clinical license, one year of experience in case management or psychosocial services, bilingual spanish, experience with electronic documentation.
