position summarythe lvn provides direct nursing care to residents in accordance with facility policies, physician orders, and applicable regulations.
this position plays a vital role in maintaining resident safety, comfort, and clinical excellence during overnight hours.
key responsibilitiesprovide direct nursing care, including medication administration and treatmentsmonitor residents' conditions and promptly report changes to the rn or physicianaccurately document nursing care and resident status in the medical recordassist with admissions, discharges, and transfers as neededcollaborate with cnas and other care team members to ensure quality resident caremaintain compliance with infection control, safety, and regulatory standards
qualificationscurrent lvn license in good standing in the state of californiaskilled nursing or long-term care experience preferredstrong clinical judgment, time management, and communication skillsability to work independently and reliably during noc shift hours
